Your Journey to Doravic
1. Obtain Form
Download the application forms below or purchase directly from the school administration desk.
2. Submission
Return the completed files alongside required documents and proof of the application fee.
3. Assessment
Scholars take an age-appropriate entrance assessment while parents join an interactive interview.
4. Enrollment
Successful applicants receive formal admission letters. Pay the acceptance fee to lock in placement.

Verification Checklist
Please gather these certified files prior to scheduling your child's assessment block.
- ✓Completed and signed admission application form
- ✓Certified birth certificate or official age declaration
- ✓Two recent passport-sized color photographs
- ✓Most recent academic report card from previous school
- ✓Official transfer certificate (for transferring secondary students)
- ✓Photocopy of parent or sponsor's valid government ID
